2017-2018 School Checklist

As part of every new year, there are a number of documents that the school and district require that parents or caregivers of each student review, sign, and return. These not only make you aware of the school’s policies but also help the school contact you in case of emergency. Please make this a priority, and return printed* signed copies of each required document listed below.

*Please TYPE your information into the Emergency Contact Form before printing out (unfortunately you will not be able to save your form). It is vital that we are able to accurately read names and numbers in case of an emergency. After printing, please be sure to sign and date the form.

ALL signed forms are due Friday, August 25, 2017.
